    Neonatal testicular tumour presenting as an acute scrotum

    Juvenile granulosa cell tumour (JGCT) is a rare benign stromal cell tumour of the testis accounting for approximately 1% of all paediatric testicular tumours. Presenting primarily as a painless testicular mass, the tumour may be associated with undescended testis, hydrocele or testicular torsion. Abnormal karyotype has also been described. We describe an unusual case of a neonatal juvenile granulosa cell tumour presenting as an acutely swollen, tender testis, originally diagnosed as an acute hydrocele. We describe the management and review the literature pertaining to this rare differential diagnosis of a neonatal acute scrotum.Keywords: acute scrotum, granulosa cell tumour, neonatal, testicular tumou

    Predictors of breastfeeding duration among women in Kuwait: results of a prospective cohort study

    The purposes of this paper are to report the prevalence of breastfeeding to six months among women in Kuwait and to determine the factors that are associated with the duration of breastfeeding. A cohort of 373 women recruited from maternity wards in four hospitals in Kuwait city were followed from birth to 26 weeks postpartum. The association of any and full breastfeeding duration and predictor variables were explored using multivariate Cox’s proportional hazards models. At six months, 39% of all infants were receiving some breast milk and only 2% of infants had been fully breastfed to 26 weeks. Women born in other Arab countries were less likely to discontinue breastfeeding than women born in Kuwait. Other factors positively associated with breastfeeding duration were level of maternal education, higher parity, infant being demand fed in hospital and a preference for breastfeeding on the part of the infant’s father and maternal grandmother. The introduction of a pacifier before four weeks of age and the mother intending to return to work by six months were negatively associated with duration. These findings present a number of opportunities for prolonging breastfeeding duration in Kuwait

    Application of Tracers in Arid Zone Hydrology

    Abstract The State of Qatar Ministry for Municipal Affairs and Agriculture is considering artificially recharging two carbonate aquifers in northern Qatar. A feasibility study comprising the drilling and testing of boreholes and a programme of injection and re-abstraction tests has been carried out at four sites. At some of these sites the only water available for injection was of similar composition to the native groundwater at the test site and Rhodamine WT fluorescent dye has been used successfully to label the injected water. Analysis of data indicate different recoveries of injected water calculated from Rhodamine WT and electrical conductivity data and different behaviours of the two tested aquifers. Numerical simulation of tracer and hydraulic data using the SWIFT III finite difference model support a complex conceptual hydrogeological model with solute transport in a multi-layered, dual porosity aquifer.     Metal-shell nanocapsules for the delivery of cancer drugs

    Cytotoxic drugs tend to have substantial side effects on healthy tissues leading to systemic toxicity, limited tolerated doses and reduced drug efficacy. A prominent research area focuses on encapsulating cytotoxic drugs for targeted delivery to cancer tissues. However, existing carriers suffer from low drug loading levels and high drug leaching both when circulating systemically and when accumulating in non-target organs. These challenges mean that only few encapsulation technologies for delivery of cytotoxic drugs have been adopted for clinical use. Recently, we have demonstrated efficient manufacture of impermeable metal-shell/liquid core microcapsules that permit localised delivery by triggering release with ultrasound. This method has the potential to improve on existing methods for localised drug delivery because it: • Encapsulates high concentrations of low molecular weight hydrophobic drugs; • Prevents leaching in non-target areas resulting in the possibility of drastically amplifying the enhanced permeability and retention (EPR) effect; • Allows triggered release via ultrasound to deliver high drug doses locally. We demonstrate here the further miniaturization of both the emulsion droplet template and the thickness of the surrounding metal shell to the nanoscale in an attempt to take advantage of the EPR effect and the excretion of nanoparticles by the hepatobiliary system

    Success-First Decision Theories

    The standard formulation of Newcomb's problem compares evidential and causal conceptions of expected utility, with those maximizing evidential expected utility tending to end up far richer. Thus, in a world in which agents face Newcomb problems, the evidential decision theorist might ask the causal decision theorist: "if you're so smart, why ain’cha rich?” Ultimately, however, the expected riches of evidential decision theorists in Newcomb problems do not vindicate their theory, because their success does not generalize. Consider a theory that allows the agents who employ it to end up rich in worlds containing Newcomb problems and continues to outperform in other cases. This type of theory, which I call a “success-first” decision theory, is motivated by the desire to draw a tighter connection between rationality and success, rather than to support any particular account of expected utility. The primary aim of this paper is to provide a comprehensive justification of success-first decision theories as accounts of rational decision. I locate this justification in an experimental approach to decision theory supported by the aims of methodological naturalism

    Nodular regenerative hyperplasia and liver transplantation: a systematic review

    Nodular regenerative hyperplasia (NRH) is a primary disease of the liver that may cause noncirrhotic portal hypertension. Common causes include autoimmune, hematologic, immune deficiency, and myeloproliferative disorders. Given the limited data regarding the development of NRH in contemporary immunosuppressive protocols and the occurrence of NRH post-liver transplantation, we systematically reviewed NRH as it pertains to liver transplantation. We performed a comprehensive search for NRH and transplantation. Nineteen studies were identified with relevant data for NRH as an indication for a liver transplant. Thirteen studies were identified with relevant data pertaining to NRH development after liver transplant. Pooled analysis revealed 0.9% of liver transplant recipients had NRH. A total of 113 patients identified with NRH underwent liver transplantation. Most series report transplants done after the failure of endoscopic banding and TIPS management of portal hypertension. Reported 5-year graft and patient survival ranged from 73%–78% and 73%–90%. The pooled incidence of NRH after liver transplant for all indications was 2.9% and caused complications of portal hypertension. Complications related to portal hypertension secondary to NRH are a rare indication for a liver transplant. NRH can develop at any time after liver transplantation often without an identifiable cause, which may lead to portal hypertension requiring treatment or even re-transplantation

    Tissue-specific interactions of TNI isoforms with other TN subunits and tropomyosins in C. elegans: The role of the C- and N-terminal extensions

    The aim of this study is to investigate the function of the C-terminal extension of three troponin I isoforms, that are unique to the body wall muscles of Caenorhabditis elegans and to understand the molecular interactions within the TN complex between troponin I with troponin C/T, and tropomyosin. We constructed several expression vectors to generate recombinant proteins of three body wall and one pharyngeal troponin I isoforms in Escherichia coli. Protein overlay assays and Western blot analyses were performed using antibodies. We demonstrated that pharyngeal TNI-4 interacted with only the pharyngeal isoforms of troponin C/T and tropomyosin. In contrast, the body wall TNI-2 bound both the body wall and pharyngeal isoforms of these components. Similar to other invertebrates, the N-terminus of troponin I contributes to interactions with troponin C. Full-length troponin I was essential for interactions with tropomyosin isoforms. Deletion of the C-terminal extension had no direct effect on the binding of the body wall troponin I to other muscle thin filament troponin C/T and tropomyosin isoforms

    Thinking About Causation : A Causal Language with Epistemic Operators

    In this paper we propose a formal framework for modeling the interaction of causal and (qualitative) epistemic reasoning. To this purpose, we extend the notion of a causal model [11, 16, 17, 26] with a representation of the epistemic state of an agent. On the side of the object language, we add operators to express knowledge and the act of observing new information. We provide a sound and complete axiomatization of the logic, and discuss the relation of this framework to causal team semantics.Peer reviewe
